Switching On

Switching on SpineMED®. First, switch on the emergency power supply down here – the button is in the middle. It will then beep briefly once.
Then we go to the tablet.
Here at the bottom right is the power switch – hold it briefly for 1–2 seconds.
Wait until the tablet boots up. Most devices can be logged in to with “user-user” – all in lowercase.
Unless you have your own user ID or password.
Otherwise, use “user-user” – and then we are on the input mask in the main field.

Patient Admission

So, today we are going to do a new patient admission.
That is: How do I create a new patient who is there for the first time?
In the main menu, go to the Patient / Patient button in the Administration area on the left.
Then we go to Add – “Add”.
As a result, I can now enter the patient in the fields above: Capital letters – Max Mustermann.
We do not have to fill in all the fields here in the input mask, but:
First name, last name.
Next, Country – he also wants that, here “Germany”.
Gender – female, male or female – let’s leave it at “male”.
Then I enter the date of birth here.
We can select the months – for example, April, April 24th.
As a result, the year of birth in increments of ten downwards – for example, 84, i.e. 1984.
Then I go to Select and have the date of birth.
We need a patient ID – it is best to use the ID that you have also stored for the patient in your practice.
We can take it here: 012345.
Physician – this is the referring doctor.
Body weight – very important: You have your conversion table there.
Always in pounds here – e.g. 180 pounds.
It also shows the kilograms in brackets at the back.
Postoperative – yes / no.
You have to click on that too.
That is: Has the patient already had an operation or not?
Date of Proseguire – is the current date, which he selects automatically.
Let’s just go to Select.
Then we have to select here: Is it a lumbar or cervical treatment?
Where is the problem? Where is the indication?
We then have the various levels in the lumbar or cervical spine area – select, for example:
Radiation for herniated disc L4/L5.
Then go to Save and we have now stored the patient in the system.

Lumbar Treatment

So, we are now going to show how to carry out a treatment on a lumbar patient – that is: treatment of the lumbar spine. In the main menu, we go to Lumbar Patient (Lombar Proseguire) – here at the top right. Then we go to Select Patient, that is, we select the patient we want to treat today, and then we have a List of Patients here. We are now using our test patient for demonstration purposes – we select them.
Then we have a mask on the left – we have to enter a few things there. Firstly, the tensile force in pounds. Here we enter the tensile force that we will use in today’s session, for example 32 pounds or 40.5 kilograms. Very important: Please ask for the VAS next – that is, the visual analogue pain scale.
Please enter it here every time – that is: You ask the patient when they come in: “Where are you today?” And he would tell you where he is in terms of pain. Then fill in the field accordingly – for example “6” – and go to Select. Then we can ask the patient to lie down on the bed: “Please sit down and lie on your back, head up here.”
It is best to ask the patient to take a seat for him here in the middle of the device – because the patient is already lying relatively well in the pelvis where he has to go later for the holder, yes. In the next step, we take the pelvic fixators and insert the first one into the openings provided on the opposite side. Then I’ll have a look with my hand – aha, now I see: That’s already relatively good. We have the spinal mark here – which should be approximately at the bottom corner. That means I can ask the patient to slide down a little further:
“Please 1–2 centimetres.” – “Okay, wonderful.” Then I take the other pelvic support and place it in here. It is important that – when we push in the pelvic supports – the device is fully open. I will then place the legs underneath so that the patient is lying comfortably here. You can take a positioning wedge here, but you can also place it on a knee roll.
Now you have to select here: the Angle Setting. That is: How do I want to treat the patient – based on the indication? Where are the complaints, where is the problem? For example, 5 degrees – then the device moves here to the desired angle setting.
Then we can fix the patient. Check again: Does it fit here? – Okay, that looks good. “Slide up a little more.” – “Okay. So.”
Then we take the pelvic strap – we place it here for slim patients so that we place it well in the middle – in such a way that we have approximately 90 degrees. Not horizontally, but really as a cross. Be careful not to squeeze the strap here when closing it. Then we can fix the pelvis nicely here. Then we turn it until it is very compact and there is no more air in between. It has to fit very compactly.
Then we go down, inflate the lordosis bladder – which is down here – three thrusts: 1, 2, 3. This gives us even more compactness in the lumbar spine area.
Then we do the rest of the input mask. We now switch on infrared in the Elvis area: Turn on Infrared Heat – so. And then we can start the treatment.
Then we go to Start. Then he always asks us for start approval: Please press the Emergency Switch once. And then for the final start approval, press Start again here – and then the treatment begins. And then I turn the monitor to the patient again at the end.
That would now be the lumbar spine treatment.

Cervical Treatment

So, we are now going to show how to carry out a treatment on the cervical spine. That is, we go to Presugers: Cervical in the right-hand area of the main menu. Then we look for the patient, go to Select Patient in the middle at the top, select our patient – for example Max Mustermann – here on Select at the bottom right.
Then we can first enter the tensile force at Tension, for example 10 pounds – on the right I have the information in kilograms.
Here too, it is important to ask about the pain scale – for example pain scale “4”, go to Select. Then we select the angle settings. That is, we now go to Select Angle and select the segment to be treated.
Most indications in the cervical spine area are C5, C6, C7 – i.e. e.g. C5/C6. 28 degrees – then the head section moves to the desired angle position. We also place a Clinex cloth between everywhere where we have skin contact. You can press it on a little like this – then it will hold directly.
When the cervical spine head section has moved to the desired angle position, we ask the patient to take a seat: “Please sit down here and lie on your back – your head here.”
I stand best with the patient, already have the head here, and guide and help the patient so that the head is also nicely in the holder here. The collar – shirt collar or something – should be outside. Remove earrings. For long hair, the patient should also use a hair tie.
It is important that the holders connect well here at the shoulder-neck triangle so that the patient is lying optimally.
Then we take the knee wedge for a comfortable positioning of the patient. We help the patient so that the legs lie on it here and the patient is now relaxed.
Then we take the forehead strap – we can also place a Clinex cloth in between. You hold it like this and then insert it into the eyelets provided for this purpose.
It doesn’t have to be super tight – but it should be tight enough to fix the head. It is important that the patient does not move their head – does not lift it, does not turn it to the left or right.
Then we have done the positioning of the patient so far. We then take the patient monitor, turn it back towards the head so that the viewing angle points nicely to the monitor. Check again whether everything fits in terms of the input. Then we go to Start, again make the start release via the Emergency Switch – and then we can finally start the device.

Data Backup

Now I’ll show you how to back up the data on the device. We go to the Administration area, to Database Management, then to Backup Database.
So – Backup Internal Drive. That is: If we click on Backup here, a data backup will be made to the internal hard drive. Database complete? Yes.
We can also – if you connect a USB stick to the tablet here – back up to an external drive (External Drive).
As soon as we connect a USB stick or a data stick, a window External Drive pops up here, and then we can save the backup to the External Drive.
But at the very least, you should back up to the Internal Drive once a month.

Switching Off

So, switching off SpineMED®:
When you have treated the last patient or the practice then closes, please go to “Logout Exit” under “Administration”, then to “Turn Off”.
Then go to “Yes” – “Shut down” – and then the system shuts down: first the SpineMed software, and then the Windows program also closes. The tablet then shuts down.
Please wait until the screen is completely black.
That is very, very important.
That is: Only when the screen is black do we go down here and switch off the UPS – that is the emergency power supply – in the middle here, and click on it once here.
Then it beeps briefly, and then it is also off.
